He was born February 27, 1969 in Burkesville, Ky the son of Ralph Smith and the late Rilda Mae Wooldridge Smith. Besides his mother he was preceded in death by an infant brother: Jerry Lee Smith

Survived by his father Ralph H. Smith and wife Sharon of Fairplay, KY

Two brothers: Terry Smith of Columbia, KY and Dwayne Smith of Columbia, KY

Two sisters: Mary Elaine Smith of Columbia, KY and Angela Renea Janes of Columbia, KY

Step brother and sister: Phillip Cravens and Heather Cravens

Also survived by several other relatives and friends

A graveside service will be held - Saturday, December 12, 2015 at 11:00 AM at the Turner Cemetery, Chance Road, Columbia, KY

Clergy officiating: Bro. Elbert Hadley

Interment in the Turner Cemetery, Chance, KY

Visiting will be at 10:30 am Saturday at the Turner Cemetery
