Columbia, KY (the Sparksville Community) passed away Monday, September

29, 2008 at 12:23 P.M. at the University of Louisville Hospital after an illness.

He is survived by two sons:

Terry Lyle Meredieth of Russell Springs, KY

Four grandchildren: Dustin Tyler Samuell, Hannah LeShay Samuell, Marcus

Allen Samuell, Kaden Scott Meredieth

A graveside service will be held at 1:00 PM Wednesday, October 1, 2008

at the Grider Cemetery in Russell County, KY

The family request visitation from 5:00 PM till 9:00 PM

on Tuesday at Stotts-Phelps-McQueary Funeral Home
