One daughter: Brandy Nicole Perkins Roth of Campbellsville, Ky.

One son: Byron Keith Perkins of Campbellsville, Ky.

One brother: Shelby Wayne Warren of Campbellsville, Ky.

one step-daughter: Dale Hill of Louisville, Ky.

Two sisters: JoAnn Caldwell of Columbia, Ky. & Sharon Pike of Elkhorn, Ky.

Preceded in death by one sister: Mary Frances Blessitt

also surviving Three grandchildren: Destin Perkins, Ravane Perkins & Hannah Perkins
